Clinics
The Consortium provides acute and chronic care services within the clinics through a committed staff of Community Health Aides and a Nurse Practitioner.
For advanced treatment, Mt. Sanford Tribal Consortium offers vision, dental, and specialty care on a contractual basis with several providers. Beneficiary enrollment information s available in the clinics.
Mentasta Lake and Chistochina clinics are located within Tribal Council facilities and are open weekdays. Please contact the clinics to schedule an appointment.
Community Health Representatives
Community Health Representatives assist with elder services and elder safety. To if you have questions about the CHR program please contact the operations manager at the administrative office 907-822-5399 ext 24.
